Lieutenant Commander Innes was a member of Fighter Squadron 114, Carrier Air Wing 11 aboard the Aircraft Carrier USS KITTY HAWK (CVA-63). On December 27, 1967, he was the pilot of a Douglas Attack Skyhawk (A-4B) over North Vietnam when his aircraft crashed into the ocean 15 miles offshore from Co Cong, North Vietnam. His remains were recovered on July 14, 1999 and identified on June 23, 2000. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
