Colonel Holmes was a member of the 20th Tactical Air Support Squadron. On May 22, 1967, he was the air controller in a Cessna Bird Dog Observation Aircraft (O-1G) on an air control mission over Quang Binh Province, North Vietnam, when his aircraft was struck by enemy fire and crashed. His remains were recovered on September 8, 1999 and identified on September 25, 2003. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
