Colonel Maddox was a member of the 20th Tactical Reconnaissance Squadron. On May 20, 1967, he was the pilot of a McDonnell Voodoo Reconnaissance Fighter (RF-101C) on a damage assessment flight 5 miles outside of Hanoi, North Vietnam, when his aircraft was hit by hostile fire and crashed. His remains were not recovered. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
