Colonel Underwood was a member of the 333rd Tactical Fighter Squadron. On March 16, 1966, he was the pilot of a Thunderchief Fighter (F-105D) on a strike mission over Lai Chau Province, North Vietnam. After he released his payload, his aircraft caught fire and he crashed. His remains were recovered on January 6, 1997 and identified on January 28, 1998. His name is inscribed on the Courts of the Missing at the Honolulu Memorial.
